Rate Submission Agreement Form
To be considered
rates must be submitted by January 15, 2007

Enter the name of your hotel (or
other establishment). This is a required field.

If you are a new
listing please enter all other pertinent
information.

If you are making
corrections to your current submission you need only to fill in the
information that needs changing including new rates for each month.

Submit the completed form to
Purchasing Services before the above deadline. If it is submitted correctly you
will receive a confirmation sheet on your screen when your
submission has been received by our office. Please print a copy for
your files.

It is not necessary for you to fax
this form to our office after submitting electronically and receiving a
confirmation form.

If for any reason you are having
difficulty submitting this form, please enter all your information
and fax it to this office at 250 387-7310 before the above
deadline. If you experience further problems please contact
